a medicine which cured a disease yesterday, cant cure it today why?

such a medicine resistance in the disease causing pathogens is known

resistance determinant r. the whole plasmid is called R FACTOR
such an R FACTOR is transferred by the methods of gene transfer such as
1.transformation
2.transduction
3.conjugation
as a result ,the donor bacteria makes the recipient resistant to the medicine and the awake goes on.
such an incidence of resistance is proportional to the use of antibiotics because they 1.destruct the normal sensitive flora
2.selection pressure by the drug
so what is the bio chemical basis of such a medicine resistance . it is 1.decreased permeability to the medicine
2.alternate metabolic pathways and
3.productions of enzymes inactivating the medicine
thus if a bacterial strain is resistant to a medicine it transfers the resistance to other bacteria and the chain goes on.....
so how to stop this chain reaction.
will the bacteria keep on become resistant to the medicine
